Important
information:
· Test
Report Forms can only be sent to nominated academic
institutions, government agencies, professional bodies and
employers.
·
Additional IELTS Test Report Forms cannot be sent more than two
years after the test date.
· If you
are requesting additional IELTS Test Report Forms after the initial
release of TRFs on the 13th day, or need additional Test Report
Forms sent outside the USA, the test center may charge an
administrative or shipping fee and will contact you directly to
request this payment.
·
Candidates requesting additional Test Report Form(s) from tests
held at the following USA test center only:
- Seattle, WA (US University of
Washington)
- Dallas, TX (US Southern Methodist
University)
- Las Vegas, NV (ELS)
- Honolulu, HI (ELS)

within the two-year period for which the
results remain valid, should apply directly to IELTS USA at
IELTS@ieltsusa.org.
Please fully complete this form. For
identification your emailed application must be accompanied by a
scan of your passport which clearly shows your photograph, sent to
IELTS@ieltsusa.org.

Applications that are not accompanied by a
scanned copy of a passport will be rejected. Your application
will take up to 10 working days to process once all of the required
information has been received, and does not include delivery time
which varies according to distance and country. All TRFs to
be sent inside and outside the USA will be sent by USPS.
